Structured Family Caregiving

Brief description of services/ programs:

Clinical oversight, coaching/support provided to family caregivers via Care Teams made up of RNs and licensed Social Workers. Caregivers also receive a daily stipend for their commitment. Individuals requiring assistance with activities of daily living who receive Medicaid benefits are eligible for the program. The purpose of the program is to help individuals keep their care in the home for as long as possible by coaching and supporting a family member to be their caregiver. Daily stipends are paid out bi-weekly totaling $900-$1200/mo depending on the level of care the individual requires. This level is determined by the local Area Agency on Aging.
