Wraparound Program Access Site Information

 

Overview:


The Community Mental Health Wraparound (CMHW) and Community Mental Health Initiative (CMHI) programs are designed to provide youth, with serious emotional disturbances (SED) with intensive, home and community based wraparound services. These programs are intended to supplement the youth’s current services by using an individualized, family driven, and strengths-based approach. Clients who are approved for these programs will receive a Wraparound Facilitator and could also receive a Habilitation Specialist, Training & Support for Unpaid Caregivers and Respite. Those clients in the CMHI program may also be eligible for additional services.

 

Qualifying Criteria:


In order to qualify for CMHW children must

  • Be eligible for Medicaid

  • Be between the ages of 6-17

  • Have at least two qualifying mental health diagnoses

  • Have a qualifying score on the Child and Adolescent Needs and Strengths Assessment


In order to qualify for CMHI children must

  • Be between the ages of 6-17

  • Have no open DCS or Probation case

  • Have at least two qualifying mental health diagnoses

  • Have a qualifying score on the Child and Adolescent Needs and Strengths Assessment


 

How does it work:


Once approved for the program, your family will choose a Wraparound Facilitator. This will be the person to work with the family as a whole. The wraparound facilitator will meet with you to learn your family story and family vision, the strengths of your child and family as well as what challenges you may be facing. The wraparound facilitator will then work with you to create a plan of care and to develop a child and family team. This team will consist of those in your life who have been helpful to your family and who care about you. Once you have established a plan, your wraparound facilitator will work to find resources and other services for your family. You will likely be set up with a Habilitation Specialist who will be able to work one on one with your child on building social skills, coping skills, etc. The wraparound facilitator and habilitation specialist will work with you and your child on a weekly basis. Your child and family team will meet at least one time per month to discuss progress, goals, etc.
